Medical Case Management

The need for more extensive involvement from a case management professional is needed when dealing with the many issues facing an individual who has sustained a catastrophic or serious injury.

Our team of brain injury/catastrophic specialists have developed a rapport with many treating specialists and community resources providing brain injury related services. Our intervention typically begins prior to discharge, working with the hospital treatment team in preparing for discharge and the arranging of appropriate community services. In a catastrophic case where return to work may not be the main issue, we work with the insurer, the family, and the care providers to assure cost effective quality care and the attainment of mutual goals. Our team working with these severe injuries has extensive experience and they are continually upgrading their knowledge base.

Our intervention with these injuries may include task assignments or full file
management, pertaining to both the adult and paediatric population, in the following areas:

  • Discharge Planning
  • Hospital Liaison With In-patient Team
  • Education to Client, Family, School, Employer and Community
  • Coordination of Discharge Recommendations
  • Forming of Community Based Treatment Team
  • Community/Home Re-integration
  • Functional Restoration Programming
  • Return to Work Programming
  • Future Care Cost Analysis
